Survey InformationWhat is tell us all about?We really want to hear your views. The purpose of the tell us survey is to gather information from members of the disability community (including people who live with a disability, their family members, friends, and advocates) about key issues they feel today. We are also interested in the views of people involved in service provision. Your participation is voluntary – you don’t have to be involved if you don’t want to. If you do choose to participate, We will ask you some questions, which are on the tell us survey questionnaire. There are two types of questionnaire. You can complete the questionnaire yourself and hand it back to us before you leave, or we can write down your answers for you if you prefer. You can withdraw from participating at any time during the questionnaire - no problem. What will the information be used for?The results from the tell us survey will be used by Julia Farr to promote general community awareness of the key issues felt by people in the disability community. This may include approaches to policy makers, the media, and disability interest groups. Julia Farr may also publish the material for a wider audience. Julia Farr will also distribute the results to those participants who request it. Potential benefitsOur aim is that the information gathered in the tell us survey will help grow awareness of issues in disability, and that it can be an important reference for policy makers and other leaders in the wider community, so that they take steps to improve choice and inclusion for people with a disability and their families. We are determined to make sure the information can make a difference. Anything else?Please feel free to ask any questions about anything that seems unclear to you. The representative from Julia Farr has explained the survey to me and answered my questions. I understand that I will be asked questions as part of the survey, and that the survey is to help grow general community awareness of disability issues. If I don't participate, there will be no penalty or loss of rights. I can stop participating in the survey at any time, even after I have started, and that if I do I will not be subjected to any penalty or discriminatory treatment. I understand that any information or personal details gathered in the course of this survey are confidential and that neither my name nor any other identifying information will be used or published without my written permission.
